Behaviours - Do you have problems or difficulties with:

- once concentrating, if you are distracted, does it take effort to regain your concentration?

Orientation

Lighting

Trauma
- have you ever suffered any significant trauma (physical, mental, emotional, sexual, birth, medical, or other kind of trauma)?

Stress
- on a scale of 1-10 (10 being stressed right out) do you consider your stress level to be higher than 5?
